As our network continues to grow, we remain committed to expanding our mobile outreach, making essential healthcare and wellness services more accessible to those who need them most.
We regularly serve individuals who face barriers to accessing healthcare. For many, our mobile services provide one of the few opportunities to receive essential health and wellness care.
Our mobile services offer a flexible, convenient, and accessible way to deliver care to individuals in their homes, care facilities, and communities. We coordinate our mobile healthcare teams to travel directly to our patients, helping ensure they receive the care they need wherever they are.
Appointments are available through our online form. We invite you to submit an appointment request to make an appointment, or request one online. Online appointments are not immediate and may take up to 24 hours to be confirmed.
Our partners are majority of the time registered, however coverage will depend on your health insurance. We ask you to consult with your Mobile Health and Wellness partner to learn more.
At this time our partners are mobile, so it isn’t possible to accept walk-ins. We highly recommend appointments so that we can come directly to you.
